| 目的 分析布鲁杆菌性脊柱炎（brucellar spondylitis, BS）的临床诊断及疗效。方法 选择我院2010年1月至2013年5月诊治的BS患者31例，记录患者临床症状与体征，行红细胞沉降率（ESR）、超敏C
反应蛋白（hypersensitive C-reactive protein，hs-CRP）、血清凝集试验等实验室检查，并行X线、MRI检查。所有患者给予多西环素、利福平、链霉素联合药物治疗，其中保守药物治疗27例，联合手术治疗4例（其中1例为保守治疗无效后行手术治疗），采取腰椎后路植骨内固定+前路病灶清除植骨融合术。治疗后复查以上检查指标，并评判治疗效果。结果 术后随访平均13个月，无临床复发，全部患者均获得满意效果。保守治疗患者于标准抗菌治疗后2～7 d内腰痛症状缓解。结论 通过标准抗菌治疗及必要时联合手术治疗BS疗效满意。 |

| Objective To study the clinical diagnosis and curative effectiveness of brucellosis spondylitis. Methods From Jan. 2010 to May 2013, 31 patients with brucellosis spondylitis were treated, and clinical symptoms and signs were recorded. ESR and hypersensitive C-reactive protein were measured, and serum agglutination test was done. The X ray and MRI examinations
were performed. All patients were given doxycycline, rifampin and streptomycin. Twenty-seven patients were subjected to conservative treatment, and 4 cases were given surgical procedures including the posterior lumbar bone graft and internal fixation with anterior debridement and ilium bone grafting. Results Thirty-one patients were followed-up for an average of 13 months, and no clinical recurrence occurred. All patients obtained satisfactory effectiveness. The lumbago symptom was alleviated 2-7 days after the standard antibiotic therapy in the patients receiving the conservative treatment. Conclusion Brucellosis spondylitis can get satisfactory curative effectiveness by standard antibiotic therapy or combined with surgical operation when necessary. |
