目的 探讨多学科生物心理社会康复(multidisciplinary biopsychosocial rehabilitation,MBR)方案对脊柱病人术后恐动症及相关康复指标的影响,为临床康复提供科学依据。方法 采用方便抽样方法,将2023年9月至2024年2月我院收治的54例拟行脊柱手术的病人纳入对照组,2024年3月至8月收治的54例病人纳入观察组。对照组实施脊柱术后常规护理和康复训练,观察组在对照组的基础上实施脊柱术后恐动症MBR方案。在入院时、术后24 h、术后1周及术后1个月时,评估两组病人的Tampa恐动症量表(TSK)评分、恐惧逃避信念、功能锻炼依从性、日常生活自理能力及健康状况。结果 有11例病人在出院后的随访过程中脱落,最终,观察组纳入49例,对照组纳入48例。术后24 h、术后1周和术后1个月,观察组病人的TSK评分和恐惧逃避信念评分均显著低于对照组,差异有统计学意义(P<0.05);功能锻炼依从性、日常生活自理能力和健康状况均显著高于对照组,差异有统计学意义(P<0.05)。结论 MBR方案能有效缓解脊柱病人术后恐动程度,降低恐惧逃避信念,提高功能锻炼依从性和日常生活自理能力,并改善其健康状况。 |

Objective To investigate the impact of a multidisciplinary biopsychosocial rehabilitation (MBR) program on kinesiophobia and related rehabilitation indicators in patients after spinal surgery, providing a scientific basis for clinical rehabilitation. Methods Using convenience sampling method, 54 patients who were scheduled to undergo spinal surgery in our hospital from September 2023 to February 2024 were included in the control group, and 54 patients who were admitted from March to August 2024 were included in the observation group. The control group received routine postoperative care and rehabilitation training after spinal surgery. The observation group, in addition to the routine care, implemented a MBR program targeting kinesiophobia in patients after spinal surgery. Assessments of Tampa scale of kinesiophobia (TSK) score, fear-avoidance beliefs, compliance with functional exercises, self-care ability in daily living, and overall health status were conducted preoperatively, 24 h postoperatively, 1 week postoperatively, and 1 month postoperatively for both groups. Results A total of 11 patients dropped out during the follow-up process after discharge, and ultimately, 49 patients were included in the observation group and 48 patients in the control group. At 24th h, 1st week, and 1st month postoperatively, the observation group had significantly lower TSK scores and fear-avoidance belief scores than the control group (P<0.05). Compliance with functional exercises, self-care ability in daily living, and overall health status scores were significantly higher in the observation group than in the control group (P<0.05). Conclusion The MBR program is effective in alleviating kinesiophobia, reducing fear-avoidance beliefs, and improving compliance with functional exercises, self-care ability in daily living, and overall health status in patients after spinal surgery. |
