目的 探讨唑来膦酸联合短节段骨水泥强化椎弓根螺钉内固定术治疗Ⅱ、Ⅲ期Kümmell病的临床疗效。方法 回顾性分析2022年1月至2024年10月采用唑来膦酸联合短节段骨水泥强化椎弓根螺钉内固定术治疗的15例Ⅱ、Ⅲ期Kümmell病病人,比较术前1天及术后7天、3个月、6个月、末次随访时的Oswestry功能障碍指数(ODI)、疼痛视觉模拟量表(VAS)评分,以及术前1天、术后6个月、末次随访时的伤椎后凸Cobb角、伤椎高度及骨密度。结果 15例病人均手术成功且获得完整随访,术后7天、3个月、6个月及末次随访的VAS评分、ODI较术前明显改善,差异有统计学意义(P<0.05),术后6个月及末次随访发现伤椎后凸Cobb角、伤椎高度及骨密度较术前明显改善,差异有统计学意义(P<0.05)。结论 短节段骨水泥强化椎弓根螺钉内固定术联合应用唑来膦酸能够有效治疗Ⅱ、Ⅲ期Kümmell病。 |

Objective To explore the clinical efficacy of zoledronic acid combined with short-segment bone cement augmented pedicle screw internal fixation in the treatment of stage Ⅱ and stage Ⅲ Kümmell's disease. Methods A retrospective analysis was conducted on 15 patients with stage Ⅱ and stage Ⅲ Kümmell's disease who were treated with zoledronic acid combined with short-segment bone cement augmented pedicle screw internal fixation from January 2022 to October 2024. The Oswestry Disability Index (ODI), visual analogue scale (VAS) for pain were compared on the 1st day before the operation, 7 days, 3 months, 6 months after the operation and at the last follow-up, and the kyphotic Cobb angle of the injured vertebra, the height of the injured vertebra and bone mineral density before the operation, 6 months after the operation and at the last follow-up were also compared. Results All 15 patients had successful surgeries and complete follow-ups. The VAS scores and ODI at 7th day, 3rd month, 6th month and at the last follow-up after the operation were significantly improved as compared with those before the operation (P<0.05). The kyphotic Cobb angle of the injured vertebra, the height of the injured vertebra and bone mineral density at 6th month after the operation and at the last follow-up were significantly improved as compared with those before the operation (P<0.05). Conclusion In the treatment of stage Ⅱ and stage Ⅲ Kümmell's disease, the combined application of short-segment bone cement augmented pedicle screw internal fixation with zoledronic acid can achieve an effective therapeutic effect. |
