目的 本研究旨在量化分析半乳糖凝集素-3(Galectin-3,Gal-3)及炎症细胞因子在糖尿病(DM)合并膝骨关节炎(KOA)病人血清中的表达水平,并探讨这些生物标志物与疾病严重程度的关联性。方法 本研究纳入了2020年7月至2024年7月山西白求恩医院收治的84例KOA病人,其中DM共病48例(DM+KOA组),不合并DM者36例(KOA组),纳入同期因膝关节韧带损伤或半月板损伤行关节镜诊治术的43例作为对照组。采用酶联免疫吸附试验(ELISA)检测血清中Gal-3和促炎、抗炎细胞因子水平。使用西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评估疼痛、僵硬和功能衰退情况。结果 与KOA组相比,DM+KOA组血清中Gal-3及多种促炎细胞因子水平显著降低(P<0.05);WOMAC评分显著升高(P<0.05)。Gal-3与白介素(IL)-12、IL-10等炎症细胞因子水平呈显著正相关(P<0.05)。Gal-3和各炎症细胞因子在轻、中、重度KOA病人血清中的水平逐步上升,差异有统计学意义(P<0.05)。结论 Gal-3与多种炎症因子参与KOA发病机制,可能作为KOA的早期诊断和治疗靶点,DM在调节炎症反应中具有潜在作用。 |

Objective To quantitatively analyze the levels of galectin-3 (Gal-3) and inflammatory cytokines in the serum of patients with diabetic knee osteoarthritis (KOA), and to explore the association of these biomarkers with clinical characteristics of KOA. Methods This study included 84 patients with KOA admitted to Shanxi Bethune Hospital from July 2020 to July 2024, including 48 patients with diabetic mellitus (DM) (DM+KOA group) and 36 KOA patients without DM (KOA group). A total of 43 patients who underwent arthroscopic diagnosis and treatment for knee ligament injury or meniscus injury during the same period served as the control group. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of Gal-3 and pro-inflammatory and anti-inflammatory cytokines in serum. The Western Ontario and McMaster University (WOMAC) Osteoarthritis Index was used to assess pain, stiffness, and functional decline. Results Compared with the KOA group, the levels of Gal-3 and various pro-inflammatory cytokines in the serum of the DM+KOA group were significantly reduced (P<0.05); the WOMAC score significantly increased (P<0.05). Gal-3 showed a significant positive correlation with inflammatory cytokines, particularly interleukin (IL)-12 and IL-10 (P<0.05). The levels of Gal-3 and inflammatory cytokines gradually increased in the serum of mild, moderate, and severe KOA patients, with statistically significant differences (P<0.05). Conclusion Gal-3 and inflammatory factors are involved in the pathogenesis of KOA, and may serve as early diagnostic and therapeutic targets for KOA. DM has a potential role in regulating inflammatory response. |
