目的 探讨关节镜下肩袖修复术后应用鲑鱼降钙素对肩袖损伤病人肩关节功能恢复的影响。方法 选取2023年3月至2024年3月于本院就诊的60例肩袖损伤病人,随机分为研究组(30例)和对照组(30例)。研究组在肩袖修复术后第1天开始应用鲑鱼降钙素鼻喷剂,每日给药200 IU,持续4周;4周后隔日给药200 IU,再持续4周。对照组无特殊干预。比较两组术后4周、8周的美国加州大学洛杉矶分校(UCLA)肩关节评分和肩部疼痛视觉模拟量表(VAS)评分,术后12周的肩关节被动前屈和外展活动度(ROM)及术后12周MRI影像学评估。结果 研究组术后4、8周的UCLA评分[(23.0±3.3)分、(26.0±2.1)分]均显著高于对照组[(20.3±3.9)分、(23.5±3.0)分],VAS评分[(4.2±1.3)分、(2.8±1.2)分]均低于对照组[(5.5±1.5)分、(3.9±1.4)分],差异有统计学意义(P<0.05)。研究组术后12周的肩关节前屈和外展ROM(149.4°±9.1°、142.6°±7.2°)大于对照组(140.6°±13.8°、136.0°±8.4°),差异有统计学意义(P<0.05)。术后12周MRI复查示两组修复肌腱无再撕裂,锚钉无松动,锚钉周围骨质无明显骨髓水肿信号。两组Sugaya分型的差异无统计学意义(P>0.05)。结论 鲑鱼降钙素在肩袖修复术后疼痛缓解、促进肩关节功能恢复方面,均具有一定的作用。 |

Objective To investigate the effect of salmon calcitonin on shoulder joint function recovery in patients after arthroscopic rotator cuff repair. Methods In total, 60 patients with rotator cuff injuries treated in our hospital from March 2023 to March 2024 were randomly assigned to study group (30 cases) and control group (30 cases). Starting on the first postoperative day, the study group received a nasal spray of salmon calcitonin at a dose of 200 IU per day for 4 weeks, followed by 200 IU every other day for an additional 4 weeks. The control group received no special intervention. The University of California at Los Angeles (UCLA) shoulder score and visual analogue scale (VAS) score at 4th and 8th week post-surgery, shoulder joint passive range of motion (ROM) in flexion and abduction at 12th week, MRI imaging assessment at 12th week post-surgery of the two groups were compared. Results The UCLA scores of the study group at 4th and 8th week postoperatively (23.0±3.3 and 26.0±2.1, respectively) were significantly higher than those of the control group (20.3±3.9 and 23.5±3.0), VAS scores in the study group at 4th and 8th week postoperatively (4.2±1.3 and 2.8±1.2, respectively) were significantly lower than those in the control group (5.5±1.5 and 3.9±1.4), with statistical significance (P<0.05). At 12th week, the ROM in flexion and abduction in the study group (149.4°±9.1° and 142.6°±7.2°, respectively) was greater than that of the control group (140.6°±13.8° and 136.0°±8.4°), with statistical significance (P<0.05). MRI at 12th week postoperatively showed no re-tear of the repaired tendons, no anchor loosening, and no significant bone marrow edema around the anchors in either group. There was no significant difference between the groups in Sugaya classification (P>0.05). Conclusion Salmon calcitonin has a certain effect on pain relief and promotes functional recovery of the shoulder joint following rotator cuff repair. |
