目的 探讨富血小板血浆(PRP)联合膝关节镜技术治疗前交叉韧带(ACL)断裂的分子机制。方法 随机选取2021年1月至2022年2月在我院住院治疗的40例ACL断裂病人(观察组),在PRP联合膝关节镜技术下利用自体半腱肌和股薄肌行ACL重建治疗,收集病人术前术后的外周血样本。同期收集40例健康对照人群的外周血样本作为对照组。磁共振成像(MRI)观察病人术后愈合情况。利用酶联免疫吸附试验(ELISA)检测白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平;实时荧光定量逆转录聚合酶链反应(RT-qPCR)和蛋白免疫印迹试验(Western blot)检测自噬相关蛋白(LC3B、p62和Beclin1)、表皮生长因子受体(EGFR)、成纤维细胞生长因子(FGF)、血小板衍生生长因子(PDGF)和血管内皮生长因子(VEGF)的表达;Western blot检测ERK和MEK信号通路的活化。结果 重建治疗术后MRI检查发现移植物呈均一信号,提示移植物愈合良好。与对照组比较,观察组病人术前IL-1β、IL-6、TNF-α和p62水平显著升高(P<0.05),LC3B、Beclin1、EGFR、FGF、PDGF和VEGF的表达显著降低(P<0.05),ERK和MEK信号通路被抑制。与术前比较,观察组术后IL-1β、IL-6、TNF-α和p62水平显著降低(P<0.05),LC3B、Beclin1、EGFR、FGF、PDGF和VEGF的表达显著升高(P<0.05),ERK和MEK信号通路显著激活。结论 PRP联合膝关节镜技术治疗ACL断裂可能通过自噬、ERK和MEK信号通路促进组织修复。 |

Objective To explore the molecular mechanism of the treatment of anterior cruciate ligament (ACL) ruptures with platelet-rich plasma (PRP) combined with arthroscopic techniques. Methods A total of 40 patients with ACL ruptures in our hospital from January 2021 to February 2022 were randomly selected as the observation group. Autologous semitendinosus and gracilis tendons were used for ACL reconstruction under PRP combined with arthroscopic techniques. Peripheral blood samples were collected from patients before and after surgery. Peripheral blood samples were collected from 40 healthy control individuals as the control group during the same period. Postoperative healing was observed using magnetic resonance imaging (MRI). The levels of interleukine-1 beta (IL-1β), interleukine-6 (IL-6) and tumor necrosis factor-α (TNF-α) were measured using enzyme-linked immunosorbent assay (ELISA). The expression of autophagy-related proteins (LC3B, p62 and Beclin1), epidermal growth factor receptor (EGFR), fibroblast growth factor (FGF), platelet-derived growth factor (PDGF) and vascular endothelial growth factor (VEGF) were detected by real-time quantitative reverse transcription PCR (RT-qPCR) and Western blotting. The activation of ERK and MEK signaling pathways was assessed by Western blotting. Results Postoperative MRI scans showed uniform signals in the grafts, indicating good healing. Compared with the control group, the levels of IL-1β, IL-6, TNF-α and p62 in the preoperative patients were significantly increased (P<0.05), the levels of LC3B, Beclin1, EGFR, FGF, PDGF and VEGF significantly decreased (P<0.05), and the ERK and MEK signaling pathways were inhibited. Compared with preoperative levels, there was a significant reduction in the levels of IL-1β, IL-6, TNF-α and p62 (P<0.05), the expression of LC3B, Beclin1, EGFR, FGF, PDGF and VEGF significantly increased (P<0.05), and the ERK and MEK signaling pathways were significantly activated. Conclusion The treatment of ACL ruptures with PRP combined with arthroscopic techniques may promote tissue repair through the activation of autophagy, ERK, and MEK signaling pathways. |
