目的 提出一种用于评估膝关节内积液量的体格检查试验——髌上挤压试验(superior patellar squeezing test,SPST),并评估其临床应用效果。方法 选取2020年4月至2021年6月经北部战区总医院诊治的87例病例,纳入病例均进行髌上挤压试验和浮髌试验。测量本组病人的健患侧周径差,通过患肢MRI检查量化关节内积液量,以评估两种检查方法的准确性。结果 纳入的87例均顺利进行了体检和影像学检查。髌上挤压试验:69例阳性,18例阴性(18例1级,21例2级,29例3级,19例4级);浮髌试验:64例阳性,23例阴性。根据膝关节MRI矢状位片精确评估膝关节内积液量,髌上挤压试验的灵敏度和准确性分别为97.14%、96.55%,高于浮髌试验的81.58%和81.61%。结论 髌上挤压试验可作为一种评估膝关节内积液量的体检方法,其灵敏度和准确性均令人满意。 |

Objective To propose a physical examination test for the evaluation of fluid in the knee joint—Superior Patellar Squeezing Test (SPST) and to evaluate its clinical application. Methods A total of 87 cases diagnosed by General Hospital of Northern Theater Command from April 2020 to June 2021 were selected, and all cases included underwent SPST. The accuracy of this examination method was evaluated by quantifying the amount of joint fluid accumulation through MRI examination of the patient's affected limb and the difference in circumference between the healthy and affected sides. Results All 87 included patients underwent physical examination and imaging. There were 69 cases positive for SPST results, and 18 negative (18 cases of grade 1, 21 cases of grade 2, 29 cases of grade 3, and 19 cases of grade 4). Floating patellar test revealed 64 were positive and 23 were negative. The sensitivity and accuracy of the suprapatellar compression test were 97.14% and 96.55%, respectively, based on the accurate evaluation of fluid accumulation in the knee joint using MRI sagittal films, which were higher than 81.58% and 81.61% of the floating patellar test. Conclusion The SPST can be used as a physical examination method to evaluate the amount of fluid accumulation in the knee joint, with satisfactory sensitivity and accuracy. |
