| 目的 探讨经伤椎植入同种异体骨和富血小板血浆（platelet-rich plasma，PRP）并置钉治疗胸腰椎骨折的临床疗效。方法 本研究是一项前瞻性研究，选取2019年3月至2021年3月在宣城市人民医院采用经伤椎植入同种异体骨和PRP并置钉技术治疗胸腰椎骨折病人35例，记录手术时间、术中出血量及手术并发症，比较术前术后疼痛视觉模拟量表（visual analogue scale，VAS）评分、Oswestry功能障碍指数（Oswestry disability index，ODI）、后凸Cobb角和椎体前缘高度比。结果 35例病人均随访12个月以上，手术时间为（112.97±17.09） min，术中出血量为（104.86±25.82） mL，所有病人无切口并发症、断钉断棒发生。病人术后1周、末次随访VAS评分及ODI指数均较术前明显改善（P＜0.05）；术后1周、末次随访后凸Cobb角较术前明显变小（P＜0.05），椎体前缘高度比较术前明显增大（P＜0.05），但术后1周及末次随访时数据两两比较，差异无统计学意义（P＞0.05）。结论 经伤椎植入同种异体骨和PRP并置钉治疗胸腰椎骨折可以有效恢复和维持伤椎高度及后凸Cobb角，疗效确切，具有较好的临床价值。 |

| Objective To investigate the clinical effect of traumatic vertebral nail placement with allogeneic bone and platelet-rich plasma (PRP) in the treatment of thoracolumbar fracture. Methods This study is a prospective study. From March 2019 to March 2021, 35 patients with thoracolumbar fracture were treated by traumatic vertebral nail placement with allogeneic bone and PRP in Xuancheng People's Hospital. The operation time, intraoperative blood loss and surgical complications were recorded. Visual analogue scale (VAS), the Oswestry disability index (ODI), kyphotic Cobb angle and percentage of anterior vertebral height were compared before and after operation. Results All the 35 patients were followed up for more than 12 months. The operation time was (112.97±17.09) min, and the amount of intraoperative bleeding was (104.86±25.82) mL. All patients had no incision complications or broken nails or rods. The VAS score and ODI of patients at 1st week after operation and at the last follow-up were significantly improved as compared with those before operation (P＜0.05), the Cobb angle of patients at 1st week after operation and at the last follow-up was significantly smaller than that before operation (P＜0.05), and the percentage of anterior edge height of patients at 1st week after operation and at the last follow-up was significantly higher than that before operation (P＜0.05). There was no significant difference between the two at 1st week after operation and at the last follow-up (P＞0.05). Conclusion The treatment of thoracolumbar fracture by traumatic vertebral nail placement with allogeneic bone and PRP can effectively restore and maintain the height of injured vertebra and the kyphotic Cobb angle, with definite effect and good clinical effect. |
