      目的 研究颈椎退变病人不同体位下的甲状软骨与颈椎节段的对应关系。方法 搜集2018年1月至2018年7月100例行颈前路手术病人的术前X线影像资料,测量甲状软骨前缘中点对应的颈椎节段,描述颈椎不同位置下对应节段的分布关系,并分析影响其变化的相关因素。结果 中立位状态下,甲状软骨对应的节段主要分布在C5~C6范围,占93.0%(93/100);过伸位状态下,甲状软骨对应的节段主要分布在C4/5~C5/6范围,占82.0%(82/100)。影响甲状软骨对应节段的最主要因素是性别。颈椎过伸位下,男性病人甲状软骨对应节段主要分布在C5和C5/6,占78.6%(44/56);女性病人甲状软骨的对应节段主要分布在C4/5和C5,占72.7%(32/44);女性甲状软骨对应节段要高于男性。上述指标比较,差异具有统计学意义(P均<0.05)。结论 颈椎不同位置下甲状软骨的对应节段存在差异,随着颈部的后伸运动,甲状软骨对应的颈椎节段呈上移的规律;甲状软骨的位置存在性别差异,女性甲状软骨对应节段的位置相对较高。

      Objective To study the relationship between thyroid cartilage and cervical vertebral segment under different positions. Methods Preoperative X-ray images of 100 patients undergoing anterior cervical surgery were collected from January 2018 to July 2018, the cervical segments corresponding to the midpoint of the anterior edge of thyroid cartilage were measured to describe the distribution of the corresponding segments in different postures, and the related factors affecting the changes were analyzed. Results The corresponding segments of thyroid cartilage were mainly distributed in the range of C5-C6 in neutral position, accounting for 93.0% (93/100); the corresponding segments of thyroid cartilage were mainly distributed in the range of C4/5-C5/6 in hyperextension position, accounting for 82.0% (82/100). Sex was the most important factor affecting the corresponding segment of thyroid cartilage. The distribution of thyroid cartilage corresponding to the cervical vertebral segment was different between male and female. The results showed that the corresponding segment of thyroid cartilage in female was higher than that in male, and there was statistically significant difference. Conclusion The cervical segment corresponding to thyroid cartilage was different in different positions. With the extension of the neck, the cervical vertebral segment corresponding to thyroid cartilage moved up. There are gender differences in the location of thyroid cartilage, and the location of thyroid cartilage in female is relatively high.
