| 目的 探讨和评价带线锚钉加强带结合改良Broström重建术修复距腓前韧带（anterior talofibular ligament, ATFL）断裂所造成的慢性踝关节外侧不稳的方法和疗效。方法 回顾性分析2014年6月至2015年6月我院采用带线锚钉加强带结合改良Broström重建术修复因ATFL断裂造成慢性踝关节不稳13例患者资料，男7例，女6例；年龄为17~32岁，平均为（25.50±4.37）岁。均有踝关节外侧轻度疼痛、反复扭伤病史和6个月以上的陈旧性损伤。术后1周在踝关节支具保护下行早期功能锻炼，采用美国足踝外科协会（American Orthopaedic Foot and Ankle Society, AOFAS）踝-后足评分系统对手术前后踝关节功能进行评估。结果 所有患者术后均获得随访，随访时间为3~12个月，平均为6个月。术后末次随访AOFAS踝-后足评分平均为（90.54±3.28）分较术前的（62.00±4.47）分明显改善，差异有统计学意义（P＜0.05）。结论 应用带线锚钉加强带技术结合改良Broström重建术修复踝关节ATFL，可增强踝关节稳定性，结合早期功能锻炼，改善踝关节功能，达到理想的治疗效果。 |

| Objective To evaluate the therapeutic effects of anterior talofibular ligament (ATFL) rupture reconstruction by tape augmented Broström procedure, which can lead to instability of the ankle joint. Methods From June 2014 to June 2015, 13 cases of chronic ankle instability caused by ATFL rupture was studied, of which 7 were male and 6 were female, with an average age of 25.50±4.37 years (range: 17-32 years). All the patients suffered from chronic pain and frequently sprained more than 6 months. Suture anchors were used to repair ATFL rupture, but the sutures were not cut and used as tape augmentation to rebuild the lateral stability of ankle. The weight-bearing rehabilitation with brace was carried out 1 week after operation. The therapeutic effects were evaluated by American Orthopaedic Foot and Ankle Society (AOFAS) scores. Results The patients were followed up from 3 to 12 months (mean 6 month). The AOFAS scores were increased from 62.00±4.47 to 90.54±3.28 (P＜0.05). Conclusion Tape augmented Broström procedure provides a stable fixation for patients with chronic lateral ankle instability to return to their normal functional activity level. It enables patients to do early functional exercise, which would give a chance to patients for the better ankle function. |
