      目的 评价在关节镜辅助下有限切口治疗髌骨骨折的效果。方法 对2005年5月至2010年9月收治的81例髌骨骨折患者进行回顾性分析,分为两组:43例髌骨骨折患者在关节镜辅助下通过有限切口进行骨折诊断和治疗,38例患者行常规髌骨骨折手术治疗。两组患者年龄、性别、骨折性质等基本情况比较,差异无统计学意义(P>0.05)。对比分析两组患者的切口长度、手术时间、术后肿胀程度、愈合时间和关节功能恢复情况。结果 与常规组相比,关节镜组手术切口小,手术时间相对短,术后组织肿胀轻,平均住院日和平均愈合时间明显缩短,胥少汀膝关节功能为优的比例明显增高,Lysholm评分的改善程度明显增高[(89.43±12.43)分/(82.12±13.12)分],错误位置觉显著降低[(2.06±0.56)分/(2.75±0.73)分]。以上改善与常规组相比,差异均有统计学意义(均P<0.05)。结论 在目前微创技术发展时期,应用关节镜辅助切开复位内固定手术治疗髌骨骨折,对于减轻术中并发症及提高患者术后恢复效果明显。

      Objective To observe the therapeutic effect of arthroscopy assisted minimally invasive surgery in treating patellar fracture. Methods A retrospective analysis was performed on 81 patients with patellar fracture enrolled in our department between May 2005 and September 2010. Forty-three patients with patellar fracture were subjected to the arthroscopy assisted minimally invasive surgery, and other 38 cases were given routine surgery as controls. There was no significant difference between the two groups in foundational data such as age, sex and types of fracture (P>0.05). Statistical analysis was performed to compare the size of section, duration of operation, wound swelling, healing time and the recovery of joint function. Results As compared with control group, minimally invasive surgery assisted by arthroscopy could reduce the section size and shorten operation duration, alleviate the tissue edema after surgery, shorten the hospital stay and recovery time. Meanwhile, the functional recovery of knee joints was reflected by Xu Shaoting Scoring and Lysholm scoring. All displayed statistically significant differences. Conclusion The arthroscopy assisted minimally invasive surgery is a good choice to treat patellar fracture, which can reduce the complication and improve the functional recovery of patients.
